    100 mg PO bid

  • Amoxicillin/clavulanate · p.o.

    875/125 mg PO bid (or 1 g/62.5 tid per local form)

  • Azithromycin / clarithromycin · p.o.

    Azithro 500 mg day 1, then 250 mg daily ×4; or clarithro 500 mg bid
