Schedule

Evidence-based EU baseline built from widely used EU/EEA patterns and ECDC Vaccine Scheduler principles (not a single country schedule). Optional vaccines are marked as country-dependent; always verify local programme.

AgeVaccineDose
BirthHep B

Birth dose is universal in some EU/EEA programmes; at minimum recommended for infants of HBsAg-positive mothers (local policy).

Neonatal (country-specific)BCG

Offered in TB-endemic EU countries; not universal across EU.

Country-dependent; adhere to product age limits for starting/completing the series.

Country-dependent (e.g. UK and selected EU/EEA programmes).

4 monthsDTaP-IPV-Hib

Many EU/EEA programmes use a 2+1 primary schedule (2, 4, 11–12 months) for hexavalent combinations; some use 3+1 (2, 3, 4, 11–12 months).

#2
PCV

Common EU/EEA pattern is 2+1 PCV (2, 4, 11–12 months); some programmes use 3+1.

Country-dependent; if used, ensure 2-dose series (interval depends on age at start).

Country-dependent; if started ≥15 years or immunocompromised, use 3-dose schedule.

Contraindications

  • True contraindication

Do not vaccinate; re-evaluate only if diagnosis was incorrect or condition resolved per guidance.

    • Anaphylaxis to prior dose or vaccine component
    • Encephalopathy after pertussis-containing vaccine
  • Precaution

Benefits may outweigh risks; vaccinate with monitoring or specialist advice.

    • Moderate acute illness (with or without fever)
    • Recent immunoglobulin or blood product (live vaccines)
  • Postpone

Defer until recovered or interval elapsed; not a permanent contraindication.

    • Severe acute illness — vaccinate after recovery
    • High fever and systemic symptoms — postpone until stable
  • Not a contraindication (common myths)

These situations usually do not require delaying routine vaccination (unless otherwise specified by local guidance).

    • Mild acute illness (e.g., rhinitis) with low/no fever
    • Antibiotic therapy

Catch-up (basics)

  • 3-dose series: 0, 1, 6 months. Minimum intervals: dose1→2 ≥4 weeks; dose2→3 ≥8 weeks; dose1→3 ≥16 weeks; final dose should be at age ≥24 weeks (product/policy-dependent).

  • If first dose given <12 months: count as dose 0; repeat at ≥12 months and second dose ≥4 weeks later. Unvaccinated ≥12 months: 2 doses ≥4 weeks apart.

  • Primary series minimum intervals: dose1→2 ≥4 weeks; dose2→3 ≥4 weeks. Booster: ≥6 months after dose3 (programme-dependent). Pertussis-containing vaccines have upper age limits (often <7 years); after that use Td/Tdap per policy. Hib catch-up is age-dependent (≥15 months often 1 dose; 12–14 months usually 2 doses ≥8 weeks apart; <12 months needs multiple doses + booster).

  • Minimum interval between doses in infancy: ≥4 weeks (programme/product-dependent). Typical catch-up patterns: start 7–11 months → 2 doses ≥4 weeks apart + booster after 12 months (≥8 weeks later); start 12–23 months → 2 doses ≥8 weeks apart; start ≥24 months (healthy) → often 1 dose (high-risk may need more).

  • 2 doses if not immune. Minimum interval: if <13 years, give dose2 ≥3 months after dose1 (some programmes accept ≥4 weeks); if ≥13 years, dose2 ≥4 weeks after dose1. If MMR and varicella not given same day, separate live vaccines by ≥4 weeks.

  • If started before 15 years (immunocompetent): 2 doses 6–12 months apart (minimum 5 months). If started at ≥15 years or immunocompromised: 3 doses (e.g., 0, 1–2, 6 months) with minimum intervals per product/SPC.

  • If primary series incomplete: complete 3-dose tetanus–diphtheria–containing series with one dose as Tdap; then boosters every 10 years.

  • Unvaccinated adults 19–59: start standard 2- or 3-dose HepB series (product-specific); ≥60 risk-based or shared decision (CDC).

  • Through age 26 complete series; 27–45 only after shared clinical decision-making.
