Inpatient / outpatient

First line

  • Cefalexin / amoxicillin · p.o.

    Cephalexin 500 mg PO qid; or amoxicillin 500 mg tid

Alternatives

  • Clindamycin · p.o.

    300–450 mg PO tid

  • Cefazolin / ceftriaxone · i.v.

    Cefazolin 1–2 g IV q8h; or ceftriaxone 1–2 g daily

  • TMP-SMX / doxycycline (+ β-lactam if needed) · p.o.

    TMP-SMX 1–2 DS bid; or doxycycline 100 mg bid
